Cystic Nephroma: A Fine Needle Aspiration Diagnostic Pitfall

  To the Editors: We read with interest the article authored by Dr. K. T. K. Chen in the January-February issue of Acta Cytologica.1 As emphasized in the article, cystic nephroma is a benign, multilocular, cystic neoplasm that continues to pose challenges in making the correct diagnosis by fine needle aspiration (FNA). The rate of misdiagnosis may be as high; however, it may be skewed because of the tendency to report the false positive cases. However, we are adding another false positive case where the FNA interpretation of adenocarcinoma was rendered but the final histologic diagnosis turned out to be classic cystic nephroma. In addition, we would like to emphasize and present a specific cytopathologic feature that may aid in making the correct diagnosis or, at the minimum, remind pathologists of the differential diagnosis before committing a malignant diagnosis.
