| To the Editors: We recently came across an interesting cause of bilateral adrenal incidentaloma in an adult male patient. A 58-year-old male patient, a teacher by profession, presented with a history of fever accompanied by cough and cold for the previous month. He was prescribed antibiotics and antipyretics; his fever subsided, but the cough continued. The fever subsequently returned as an evening rise in temperature associated with general ill health. His general and systemic examination at that time was unremarkable. There was a past history of bleeding hemorrhoids. Routine hematologic and biochemical investigations were within normal limits. Chest radiograph showed pneumonitic infiltrations involving the right upper zone; however, a Mantoux test and sputum test for acid-fast bacilli were negative. Serum Widal, hepatitis B surface antigen and enzyme-linked immunosorbent assay for human immunodeficiency viruses 1 and 2 were also negative. Ultrasound of the abdomen and pelvis showed prostatic enlargement with significant residual urine, and the patient underwent transurethral resection of prostate and a hemorrhoidectomy. In the postoperative period, he had peaks of high fever with rigors, at which time a urine culture was positive for Escherichia coli. The patient was prescribed antibiotics; febrile peaks subsided, but low-grade fever persisted. Again, his general and systemic examination was unremarkable. | ||||||||||||||||||||
