Utility of Cell Block Preparations in Cytologic Specimens Diagnostic of Lymphoma

  To the Editors: We read with interest the recent article by Barroca et al1 reporting their evaluation of fine needle aspiration cytology (FNAC) and flow cytometry (FC) as applied to the diagnosis of lymphoproliferative disorders. We concur with their final conclusion that FNAC and FC together provide a reliable and definitive diagnosis in most instances. Indeed, the ability to diagnose lymphoma by cytologic preparations has certainly expanded with the use of ancillary studies. In a subset of cases in which FNAC and FC were not sufficient for diagnosis (or in which discordant results were seen), they reported histologic evaluation of surgically excised lesions was useful in defining the respective lesions. To this end, although cell blocks are not widely used as an adjunct in the evaluation of lymphoid lesions, they are routinely prepared in our laboratory. In this context, we have found cell block preparation of cytologic material to be extremely beneficial, with the potential for further reducing the need for surgical excision.
